Berwyn EMS provides the following options for release of medical records:

1) In-person by the patient with ID

2) Signed HIPAA medical release form with original signature 

3) Subpoena

4) Search Warrant

Please see below for detailed information on each method of release:

In Person by the patient:

Advanced request is required by emailing EMSCaptain@BerwynFireCo.org 
Once the records are available, you'll be able to obtain them in person at our Bridge Ave station.  ID is required. 

Signed HIPAA Medical Release form with original signature:

If a patient is unable to come to the station in person, a representative of the patient may pick up the records in person with an original signed HIPAA release form.

Once records are available, you will be able to obtain them in person at our Bridge Ave station. Original signed release form is required.

Subpoena:

Law firms and courts may subpoena medical records by normal procedures. All requests can be made by the following methods:

All medical requests and subpoenas associated with Civil matters must fill out this form.  Payment of $75 must be paid in advanced. 

*Any subpoenas for civil matters that require deposition or court testimony are billed at a rate of $150/hr per person. Minimum of 2 hours per person.  Zoom/remote depositions are respectfully requested in advanced.  Time for travel may be included if zoom or remote appearances are not offered.

Search Warrants and other criminal matters

Executed search warrants may be served in person at Berwyn Fire Co at 23 Bridge Ave Berwyn PA 19312.  If advanced notice is provided, records can typically be obtained within 1 hour by the on-duty Battalion Chief.
